This report is one in a set of resources describing the development of a prototype for a new digital resource, How's My Health Dashboard, which would help patients and health care providers work together to achieve health goals that are important to patients. This project was conducted by United Hospital Fund and supported by a grant from the New York State Health Foundation. This report includes findings from nine months of co-design work with patients and clinicians from a New York City primary care practice, interviews about their use of health information technology, the process of creating the content and design of the dashboard, user feedback about the prototype, and considerations of individual, organizational, and cultural factors that would facilitate its adoption. Accompanying resources include an appendix on methods that provides details about the practice and patient participants, as well as the novel co-design approaches adopted given the unique context in which the project was conducted--namely from May to December 2020, amid unprecedented changes in health care delivery and significant societal health impacts experienced by patients and health care providers.
