Last Wedn. Vomiting. Mem. lost. Did not know she had been here. 2 am reg. memory. ab. loss of mem. from 7 pm to 2 am. Now all night. No abt.
Gallstones 3 y ago. Typhoid when young. 4 childr. 1 dead. No headache. Good appet. For last few years ailing. Every year worms [ . . .] 5 y ago. Hus. died. One spring had lumbago. Special [ . . .]. Nose. This sum at Atl. City took hot baths. Got nervous, emotional. Pain in head, and ner. weakness. Dull headache in am [ . . . ] could not eat in Atl. city a little loss in weight.
gets up 3 times at night. Nose is [ . . . ] than usual. Drunk.
Exam negative. no [ . . .] arterio sclerosis. Head clear. No change in the [. . . ] To go slow and care in eating & diet. [ . . .]
