myqyuul REQUIRED OF THE FRED HUTCHINSON CANCER RESEARCH CENTER (FHCRC) General Comments The construction guidelines of the National Cancer Institute are to be followed in the development of the FHCRC facilities. Although NCI does not have the responsibility for review and approval of the construction plans, NCI representatives are willing to assist in their deve lopment and review. It is suggested that arrangements for NCL assistance be coordinated by the HEW X, Federal Engineering Construction Authority representatives, since that agency has the responsibility for review and approval of the plans. . In order to complete the RMPS review and award process before July 1; 1972, it is extremely important that the following requirements be met as indicated. - | | . To be submitted no late’ than June 1, 1972, to RMPS and FECA. I. Program Document Specifics: The program document will include a floor by floor layout %r schematic of the facility indicating what types of activities would be conducted on each floor, a description of the building and the type of equipment to be included therein. Direct submission of a cop: of the document to FECA will expedite the review process. II. Documentation of Technical and Administrative Requirements . To be subwitted no later than June 1, 1972, to RMPS. Specifics: Relevant requirements must be met including all necessary licenses, clearances, permits and approvals when required. . A. Evidence of nonprofit status of applicant and of Swedish Hospital ; Q. Compliance with HEW Civil Rights Regulation (HEW Form 441) R. Requirements for accreditation by the Joint Commission Determination of Fiscal Agent -, fo be submitted no later than June 1, 1972, to RMPS. | | Specifics: Since the grant will be made directly to the FHCRC, it must be determined if the FHCRC will be adequately staffed and organized to serve as its own fiscal agent. If not, some other established organization would have to act as the fiscal agent as the University of Washington does for the Washington/Alaska Regional Medical Program. Response to National Advisory Council, RMPS Conditions for Award A. All relevant Federal, state and local requirements concerning the expenditure of Federal funds for the construction of the proposed type of facility including all needed licenses, clearances, permits, and approvals. Specifies: (Covered in Item II above.) B. The University of Washington and Swedish Hospital formalize their relationships with the Center through written agreements. - To be submitted by May 1, 1972, to RMPS. Specifics: University of Washington--Early in February, the affili- ation agreement between the University of Washington and the FHCRC was in draft form, and was awaiting acceptance by the University Board of Directors. — Specifics: Swedish Hospital Medical Center--In a letter of January 31, 1972, to Dr. Hutchinson, Dr. Allan W. Lobb, Medical Director, Swedish Hospital Medical Center, states that ... "We clearly understand that our facilities will be required by the th Page 3 - REQUIRED OF FHCRC vi. research center. Further, the, "... Center is in a position and has the facilities at the present time to make available such services as will be required to service up to 50 beds in the new research building." The letter also states "The Board of Trustees and the medical staff reaffirm their encouragement in the development of the Fred Hutchinson Cancer Research Center and will cooperate to the best of their ability." The intent of the NAC condition was that the Board of Trustees of the Swedish Hospital Medical Center give official sanction and approval of the FHCRC and provide an affiliation agreement, which would include what services and staff would be provided by the Swedish Hospital Medical Center and what would be provided by the FHCRC. | ¢.
