The instructions to medical officers, and the forms by which they are accompanied, issued this day hy the Chief of the Bureau of Medicine and Surgery, have received my approba- tion, and I enjoin their strict observance at hospitals, navy yards, and on board all national vessels. The Bureau of, Medicine and Surgery is charged with the genera] control of every thing relating to the treatment of the sick and W’ounded of the navy, whether at hospitals, navy yards, or on board of public vessels. In its charge are included hospitals, furniture, surgical instruments and appliances, medicines, stores of all kinds, and attend- ants of every description. 2. It is the duty of the Bureau of Medicine and Surgery to keep a record of the services of all medical officers, and to’ nominate them for duty of every description. Therefore, all medical officers of the navy are directed to address their official communications for the Navy Department to the Chief of the Bureau of Medicine and Sur- gery, and they will receive all official communications from the Navy Department through his office. 3. To facilitate the duties of medical officers in the navy, and to se- cure uniformity in the accounts and records of this bureau, the official forms herein described have been adopted. All other forms relating immediately to the duties of surgeons are hereby revoked. 4. On joining a vessel for sea service, or taking charge of a hospital, or assuming the duties at any post or station, the surgeon shall receive from his predecessor all public property that may be in his charge, stated on the form A. The surgeon shall sign this return in duplicate ; one copy he shall retain, and give the original to the surgeon he may re- lieve, by whom it shall be transmitted to this bureau, and the acknow- ledgement of its receipt here will acquit him from being responsible for the property named. 5. At the expiration of the druise and return of a vessel to the United States, the surgeon shall prepare, in duplicate, a statement of all public property in his charge, according to form A, and deliver all the articles named, to the surgeon of the navy yard at which the vessel may arrive. The latter shall receipt for them on the “ return,” the original of which shall be given to the surgeon of the vessel, by whom it shall be forward- ed to this bureau ; the acknowledgment of its reception will relieva 6 him from further responsibility on this account; where there is no sur- geon to receive property thus returned, the naval storekeeper of the station, or any other person designated by this bureau, will sign the “ return,” and be governed by the above instructions to surgeons. 6. The object of this return (A) is to exhibit, half yearly, the re- ceipts and expenditures of the medical department of the navy, and to supercede the immediate necessity of survey when one medical officer relieves another, and when a surgeon returns from a cruise. It is simply a statement of the quantities or numbers of all articles in charge, show- ing those which are considered to he unfit for use, in a separate column; when this form is used as a transfer-list, the only columns to he filled up are those headed, “on hand,” and “worn out and set aside for sur- vey.” On the first day of January and on the first day of July of each year, or within ten days after these dates, the surgeon at each naval hospital, navy yard, receiving vessel or station, as well as surgeons of vessels on separate service, shall transmit this return properly filled up and signed, to this bureau. 7. A table of allowances (form B) for the medical department of vessels has been prepared as a guide to surgeons making requisitions for out-fit, without limiting them, absolutely, to the kind of articles named in it, although they will not be permitted to exceed the quantity set down, in any instance, without the special approbation of the chief of this bureau. Any less quantity may be required, and any article named may be omitted at the discretion of the surgeon. The quantities of medicines and hospital stores in the table are estimated for one year; and all other articles for the cruise or three years. 8. A requisition form (C,) to be used only for obtaining the out-fit or first supply of vessels preparing for a cruise, has been devised. It does not require the approval of the chief of this bureau; hut is to be directed to the surgeon of the navy yard where the vessel fits out. It will be, and is his duty to comply with this requisition, provided it does not exceed the limits of the allowance table, and he shall supply the articles from the stock on hand as far as practicable; and what he may not have, shall be procured from the druggist appointed on the station. It is expected, however, that every surgeon will select and examine, in person, the articles supplied on his requisition, and, before receipting for them, to the druggist or approving any bill, satisfy himself, as far as possible, that the articles are of good quality and not charged higher than at the market price. No hill for articles supplied on a surgeon’s requisition will be paid without the receipt and approval of the surgeon. 9. The quantity of every article asked for must be stated, on the re- quisition, in words, and not in figures. 7 10. For whatever articles the surgeon of a navy yard may deliver to any other surgeon he shall take a receipt, in triplicate according to the form D.; he shall forward the original to this bureau, and retain the duplicate; the triplicate will remain with the surgeon signing the re- ceipt. 11. The printed bill form (E) corresponds with the requisition form C. The receipt for the articles comprised in this bill must be signed in triplicate by the surgeon, and forwarded to this bureau for approval and payment. A quadruplicate or copy of all bills shall be retained by the surgeon who signs them, and duplicates and triplicates must be designated by him. Not more than one item of any account must be stated on one line. Surgeons shall not approve or sign any bill or account in which more than one item is stated on the same line, nor shall they approve any bill or account in which the date of pur- chase, name, price or rate of cost, number or quantity and aggregate value of each article are not distinctly and separately specified. All receipts and approvals upon accounts must be dated at the time they are written. All receipts for money must express the amount paid, in words written at full length. 12. In case the surgeon should think other articles, or larger quan- tities, than those set down in the allowance table, necessary, he may make a special requisition for them in duplicate, on the blank requisi- tion form H. and forward it to this bureau for consideration and ap- proval, stating at the same time, the reasons which induce him to ask for additions to the out-fit allowed. 13. When out of the United States, surgeons attached to vessels on separate service can replenish their supplies by drawing a requisition in duplicate, (form F,) directed to the commander and approved by him ; the bill for these supplies (forms G) made out in triplicate re- ceipted by the surgeon and approved by the commander, will be paid by the proper agent or accounting officer. 14. Surgeons of vessels attached to squadrons on foreign service shall direct their requisitions to the fleet surgeon, by whom they will be ap- proved or modified at his discretion; and they must be subsequently approved by the commander of the vessel as well as by the commander- in-chief of the squadron. It is expected that every surgeon serving in squadron will make requisitions only when in company with the flag- ship ; hut should circumstances not anticipated by him, render it ab- solutely necessary to procure supplies when absent from the fleet sur- geon, his requisition and bill will be subsequently signed by that offi- cer as well as by the commander-in-chief, in order to show that his conduct in this respect has been approved. Surgeons are held responsible for the propriety of their requisitions, and for this reason. 8 requisitions must be signed by the fleet surgeon before they are pre- sented to the commanding officers for approval ; and it will be the duty of the commanding officer to approve, if, in his opinion, the condition of the funds of the ship he commands will authorize it. 15. The Fleet Surgeon shall furnish whatever is required by any sur- geon of the squadron from the public stores in his charge, or otherwise as he may deem most expedient, with a view to economy and despatch. For whatever articles the fleet surgeon may deliver to any surgeon of the squadron, he shall take a receipt in triplicate (form D;) he shall forward the original to this bureau, retain the duplicate, and give the triplicate to the surgeon who signs the receipt. 16. Surgeons of hospitals, navy yards and receiving vessels shall make requisitions quarterly (form H) for medicines, surgical instru- ments, hospital stores, &c., arranged alphabetically and in the order of the allowance table (B) as far as circumstances allow, and forward them in duplicate to this bureau for consideration and approval. In case of necessity, surgeons will be permitted to obtain from the surgeon of the navy yard, or druggist appointed on the station, any articles during the quarter, which must be stated on a separate requisition in duplicate, and transmitted with the next subsequent quarterly requisi- tions to this bureau. Surgeons will be held strictly accountable and responsible, should they obtain any article in this way, that circum- stances do not demand, of wThich the chief of this bureau will be the judge. All requisitions shall be showm to the surgeon of the navy yard before they are filled, that he may furnish whatever he can from the stock on hand. All furnishers of supplies at stations in the United States, are required to render their bills on the forms I or U con- formably to the instructions given above. 17. In the United States, marketing, fresh provisions, fruits, &c., shall be purchased under direction of the surgeon; and on the first day of every month, the steward shall prepare a bill in triplicate (form J) embracing a statement, in detail, of the kind and quantity, price, and aggregate cost of every thing procured, to the correctness of which the surgeon will certify. The bill shall be forw'arded to this bureau for approval and payment. In cruising vessels, these daily supplies for the sick and wounded of the navy will be procured in the same manner; and the steward’s bill (form JJ,) drawn out on the first day of every month, or whenever it may be ne- cessary in consequence of the vessel being in port during a shorter period, and its correctness certified by the surgeon, shall be submitted to the commander for approval. It will then be paid by the purser, and charged by him under the proper head. The triplicate of this bill shall be forwarded by the surgeon, from vessels on separate service. 9 direct to this bureau, and from vessels attached to squadrons, to the fleet surgeon. 18. For the purpose of obtaining daily supplies of fresh fruits, meats, and such other articles as are named at the head of the form JJ, for the use of the sick and wounded, pursers of vessels are authorised, with the permission of the commander, to advance to the surgeon, small sums of money. Moneys thus advanced for the public service will be accounted for in the surgeons’ stewards’ bills, which will be the surgeon’s vouchers for the disbursement of these advances. Pursers of vessels, commissioned for sea service, are also authorised to furnish any article composing part of the ration, for the use of the sick on requisition of the surgeon, approved by the commander of the vessel. Articles thus furnished shall be charged at the valuation prescribed by the regulations of the Navy Department. 19. Bills for washing for the medical department, for repairs, or other minor contingent expenses (form U) must be signed by the sur- geon in triplicate, and, in the United States, forwarded to this bureau for approval and payment. In cruising vessels, these bills (form V) must be approved by the commander, before they can be paid by the purser. From vessels on separate service, the surgeon shall forward the triplicate to this bureau, and from vessels attached to squadrons, the triplicate shall be transmitted to the fleet surgeon. 20. Surgeons of hospitals, navy yards, and receiving vessels and of every station in the United Staff's, as well as surgeons of cruising ves- sels shall keep a register (form M) in which shall be entered, the name, rate, age, place of birth, date of admission, the name of the ves- sel or place from which received, the name of the disease or injury, date of .discharge or death, where sent, the number of days subsisted by the medical department, and the aggregate value ofthe rations stopped and credited to the naval hospital fund on account of every patient treated. Death, or desertion, or recommendation for pension will be noted under the head of remarks. This register, which is alphabeti- cally arranged, will b°corne a current record, from which several periodical reports must be derived. It is directed to be kept with scrupulous care, as it may be hereafter, the means of proving in part, individual claims to pension or gratuity. At the expiration of the cruise and return of a vessel to the United States, it shall be forwarded to this bureau ; but it will be retained at hospitals, and at other shore stations until the volume be filled when it shall be forwarded to this bureau. 21. The revenue ofthe naval Hospital fund must be carefully fostered by all medical officers. Whenever a patient is to derive his subsistence from the medical department, oris admitted into a naval hospital, the 10 surgeon shall immediately send a ration notice (form P,) to the purser or other officer on whose books the patient’s name may be, that the issue of his rations may cease ; and when the patient is discharged from the sick list, or when he is no longer to be subsisted by the medical depart- ment, the surgeon shall forward another ration notice (form Q,) stat- ing the day of his discharge, the number of days he has been subsisted at the cost of the medical department, and the value of the undrawn rations; that it, or its equivalent, may be credited to the naval hospital fund, because this fund is entitled daily to the value of a ration for every pa- tient subsisted by the medical department, no matter whether he be en- titled to a ration or not. 22. At the expiration of evmy all surgeons shall prepare an abstract (form R,) showing the name, rate, disease or injury, dates of admission and discharge, and the number of days each patient has been subsisted during the month, the aggregate value of stopped rations in each case and the total amount of the whole. If there be others than patients in a hospital or on a station, subsisted at the cost of the medi- cal department, their names, &.C., must be added to the monthly abstract, stating the value of the rations to be credited to the naval hospital fund. The surgeon is required to sign this abstract on each page. From sta- tions in the United States, and from vessels on separate service, this abstract with the duplicate ration notices shall be forwarded to this bu- reau ; and from vessels attached to squadrons the same shall be sent to the fleet surgeon. A duplicate of the monthly abstract shall be for- warded in all cases to the Fourth Auditor of the Treasury Department, for comparison with the purser’s quarterly pay roll. Pursers and sur- geons are required to settle any discrepancies which may exist~on ac- count of stopped rations at the time the ration notice (Q) is trans- mitted, as the monthly abstract will be considered as the standard for crediting the hospital fund. Therefore, the certificate of the surgeon to the “ general abstract of rations” appended to the purser’s pay roll is no longer required. 23. No patient shall be admitted into any naval hospital who is not accompanied by a hospital ticket or request, approved by the com- mander of the vessel or place to which the patient belongs, and also by the commander of the station or yard to which the hospital may be attached. This request must embrace a list of the patient’s cloth- ing, &c. and a brief history of the case written on the back or reverse of the ticket and signed by the surgeon. The hospital steward will see that the clothes list is correct; when discharged, the patient will sign a receipt for whatever may be delivered to him. 24. In all cases a strict account of the daily expenditure of hospital stores shall be kept, (form. S) which the surgeon shall examine once a week, and, if he approve, sign. These expenditure books shall be forwarded to this bureau at the end of the year. 25. Surgeons of hospitals shall report to the commanding officer of the navy yard or station to which the hospital is attached, weekly, (form N) the names, &c. of all patients in their charge; and surgeons of navy yards and of all vessels shall make a similar report, daily, to the commanding officer, (form T,) stating the name, rate, name of the disease or injury, date of admission of each patient on the sick list; and whenever a patient is discharged, he will state, under the head “ where discharged,” whether he has been discharged to hospital, from service, to be sent home, to duty, by death or by desertion, &c., mark- ing the date of discontinuance on the list in the column for “ remarks.” 26. Surgeons at stations in the United States, and surgeons of vessels on separate service shall make to this bureau a quarterly report (form O,) of the number of diseases, wounds and injuries. Surgeons of vessels attached to squadrons shall make the same report to the fleet surgeon. All surgeons are required to keep a diary or journal of practice, and for this purpose they are allowed case books and blank books. The Chief of the Bureau of Medicine and Surgery takes plea- sure in anticipating that every surgeon will endeavor to contribute to the advancement of science, by reporting in detail, cases of interest, and observations upon endemic and epidemic diseases, medical topo- graphy, &c., which may be published for the information of the profes- sion and the credit of the medical corps of the navy. 27. Surgeons at hospitals, navy yards, or other shore stations, and surgeons of receiving and other vessels shall report quarterly, (on the Ist October, Ist January, Ist April and Ist July,) the expenses of their respective charge (formK.) From all stations in the United States and vessels on separate service, this report shall be forwarded to this bureau, and from vessels attached to squadrons to the fleet surgeon. For the purpose of keeping correct accounts of expenses, journals and ledgers are supplied, and it is expected that every surgeon will be able to show what his expenditures of public moneys have been, and to compare them fairly at the expiration of his term of ser- vice with the accounts which will be kept in all cases in this bureau. 28. In cases of necessity, repairs of instruments, &c. may be made at the discretion of the surgeon; bills accruing on this account or other minor contingencies in the United States shall be forwarded to this bureau for approval and payment, (form U;) and out of the United States, they shall be subject to the approval of the commanding officer, (form V.) 29. Apothecaries, hospital stewards and matrons shall be nominated by the surgeon and appointed by the chief of this bureau, to hold their 12 appointments during his pleasure. Nurses, cooks and other attendants on the sick and wounded shall be selected by the surgeon, and hold their situations during his pleasure. At navy yards, and on board of ves- sels, the appointment or dismission of these persons cannot take place without the sanction of the commanding officer. 30. A surgeon’s steward is allowed at every hospital and on board of all vessels of every class. 31. No person shall be appointed anothecary at any naval hospital who is not a graduate in pharmacy from some respectable collega, and who is not in all respects qualified, in the opinion of the chief of this bureau, for the situation. 32. It is of great importance, especially in small vessels where there is only one medical officer, that a respectable class of persons should be employed as surgeons’ stewards. It is, theretore, (he duty of surgeons to select in preference, those persons who have had opportunities of ac- quiring some knowledge of pharmacy; who possess a good knowledge of ordinary accounts, a legible hand-writing, trustworthiness, methodi- cal and industrious ways, a good disposition and temper, and above all, temperate habits. 33, In the United States surveys on damaged or worn out articles be- longing to the medical department of the navy will be directed by this bureau, on the application of the surgeon in charge; and on foreign stations, by the fleet surgeon, with the concurrence of the commander in chief. Medical officers exclusively will be selected to discharge this duty. In their reports, surveying officers shall.direct the disposition to be made of condemned articles, always having a due regard to the in- terests and reputatio iof the service. Medicines that are unfit for use in the navy are unfit for use any where and therefore should be destroy- ed. Articles composed of cotton, linen or wool, may be applied to other uses, although no longer suitable for the purposes for which they were originally designed, and ought to be placed under the control of the sur- geon to be-used as he-spay, conducive to the interests of the service. All official communications from medical officers shall be for- warded to the Bureau of Medicine and Surgery, through the command- ing officer of the vessel or station to which they are attached, or if in squadron through the surgeon of the fleet and commander in chief. Communications on the character and treatment of diseases, on medi- cal topography and on such other subjects, as contribute to the ad- vancement of medical science, shall be forwarded directly to this bu- reau, by surgeons of hospitals, stations, or vessels on separate ser- vice, or, if in squadron, through the fleet surgeon. 35. Fleet surgeons have a general supervision over all medical affairs 13 of the squadron to which they may be attached. They are charged with receiving and forwarding (through the usual channel) all commu- nications and reports from medical officers for this bureau; and ap- proving all requisitions and bills, and procuring supplies from the depots or naval storekeepers abroad ; and with purchasing them when the re- quired supplies cannot be otherwise obtained. They will be, therefore, always careful to be informed whether there are medical supplies in store from which requisitions may be filled before purchases are made. 86. The fleet surgeon is expected to co-operate with the chief of this bureau, by close attention to the duties of his own office and to those placed under his supervision, to secure proper economy, promptness and punctuality in making and forwarding reports, returns, Bcc. He shall report quarterly, the aggregate of the diseases of the squadron, (Form O,) and the current medical expenses of the squadron, (Form K,) deduced from the returns of surgeons, (directed to him for this purpose,) which he shall also forward. 37. The law requires that all printing for the public service shall be done under contract; therefore, all blanks connected with the duties of medical officers will be furnished exclusively by this bureau; nor will bills or other communications for which blank forms have been de- vised, be received in this office, written on other paper or forms, or not in conformity to these regulations. All blanks may be obtained from surgeons of navy yards, or by application to this bureau. 38. All official letters to this bureau must be written on foolscap paper, with a blank margin of one inch and a quarter at least, on each side of the page. 39. All instructions and circulars from the Navy Department con- flicting with the above instructions are revoked. Chief of the Bureau of Med.
